We are required by law to maintain the privacy of your child’s PHI, provide you with this notice of our legal duties and privacy practices, and abide by the terms of this notice. We reserve the right to change our privacy practices and make new provisions effective for all PHI we maintain. Should our practices change, we will provide a revised notice upon request and post it on our website.
